ABSTRACT

Due to the anatomical specificity of esophagus, esophagectomy can be carried out using different approaches, such as left transthoracic, right transthoracic and transhiatal approaches. Each surgical approach is associated with a different prognosis due to the complex anatomy. The left transthoracic approach is no longer the primary choice due to its limitations in providing adequate exposure, lymph node dissection, and resection. The right transthoracic approach is capable of achieving a larger number of dissected lymph nodes and is currently considered the preferred procedure for radical resection. Although the transhiatal approach is less invasive, it could be challenging to perform in a limited operating space and has not been widely adopted in clinical practice. Minimally invasive esophagectomy offers a wider range of surgical options for treating esophageal cancer. This paper reviews different approaches to esophagectomy.

ABSTRACT

Autophagy and tumor immune escape are important biological mechanisms in the process of tumor cell proliferation and metastasis, involving multiple signaling pathways. The interaction of autophagy and tumor immune escape seriously affects the treatment and prognosis of tumor diseases. However, the correlation between autophagy and tumor immune escape is still not fully elucidated. Recent studies have shown that autophagy can affect the activity of immune cells by regulating the presentation of antigens in tumor cells, the release of cytokines, and the degradation of immune checkpoint proteins, thereby positively or negatively regulating tumor cell immune escape. The activation of autophagy in tumor cells can inhibit the activation of the innate immune sensing pathway of stimulator of interferon genes (STING)-type Ⅰ interferon (IFN-Ⅰ) to inhibit its immunogenicity and cytotoxic T lymphocytes (CTLs), which promotes tumor immune escape. While autophagy suppression can reduce the infiltration of M2 macrophages, promote the binding of natural killer group 2, member D (NKG2D) to its ligand, and inhibit the recognition of immune checkpoint proteins, thereby exerting an immune-killing effect and inhibiting tumor immune escape. Traditional Chinese medicine (TCM) has unique advantages in anti-tumor research, especially in the unilateral regulation of autophagy or improvement of tumor immunity, but the research based on the regulation of autophagy and tumor immunity by TCM is insufficient. A few studies have shown that Chinese medicine monomers and compounds can exert an anti-tumor effect by regulating cell autophagy and interfering with tumor immune escape, but there is still a lack of systematic elaboration. The present study reviewed correlation between autophagy and tumor immune escape and regulation of autophagy by Chinese medicine to interfere with tumor immune escape to provide new ideas for research on mechanism of TCM against tumor diseases and development of innovative TCM drugs against tumors.

ABSTRACT

ObjectiveTo screen the active antitumor components of Gupi Xiaoji decoction by network pharmacology and molecular docking based on the pyroptosis mediated by cysteinyl aspartate-specific protease 1 (Caspase-1) and explore its molecular mechanism in intervening in the pyroptosis of HepG2.2.15 cells through in vitro experiments. MethodThe compounds and targets of Gupi Xiaoji decoction were screened out by Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP) to obtain the corresponding gene symbols. The targets of Caspase-1 were collected from GeneCards,online mendelian inheritance in man(OMIM),PharmGKB,and TTD,and the compound-gene target regulatory network was constructed by Cytoscape. The protein-protein interaction(PPI) network was established and analyzed by STRING. The mechanism of the effective components of Gupi Xiaoji decoction on Caspase-1 was predicted by gene ontology(GO) functional enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG) pathway enrichment analyses. The molecular docking was verified with AutoDock Vina. The plasma medicated with Gupi Xiaoji Decoction was prepared and HepG2.2.15 cells were cultured in vitro. HepG2.2.15 cells were divided into a blank plasma group,a VX-765 group,a VX-765+medicated plasma group, and a medicated plasma group. After 48 hours of intervention with 15% medicated plasma, the expression and distribution of gasdermin D-N (GSDMD-N) on the surface of the cell membrane were detected by immunofluorescence staining. The release of lactic dehydrogenase (LDH), interleukin(IL)-1β,and IL-18 in the cell supernatant was measured by enzyme-linked immunosorbent assay(ELISA) kits. The expression of Caspase-1 and GSDMD-N was measured by Western blot. ResultThe mitogen-activated protein kinase 14 (MAPK14),MAPK1,protein kinase B1 (Akt1), MAPK8, V-Jun sarcoma virus oncogene homolog (JUN), and TP53 screened by network pharmacology were the main targets. The compounds 7-hydroxy-5,8-dimethoxy-2-phenyl-chromone,wogonin,rhamnazin,moslosooflavone,isorhamnetin,7-O-methylisomucronulatol,formononetin,calycosin,luteolin,quercetin,kaempferol,β-sitosterol,and baicalein screened by network pharmacology were the main active components of Gupi Xiaoji decoction. Go enrichment analysis showed that multiple biological processes were involved, including responses to oxidative stress and metal ions,ubiquitin-like protein ligase binding,and phosphatase binding. KEGG pathway enrichment analysis showed MAPK pathway,nuclear factor(NF)-κB pathway,p53 pathway, and hypoxia-inducible factor-1(HIF-1) pathway were involved. Molecular docking showed that the targets had good binding with the components. In vitro experiments displayed that compared with the blank plasma group,the VX-765 group showed weakened GSDMD-N fluorescence signal,reduced release of LDH,IL-1β,and IL-18,and declining expression of Caspase-1 and GSDMD-N(P<0.01), and the medicated plasma group showed increased GSDMD-N fluorescence signal, increased release of LDH,IL-1β,and IL-18,and up-regulated expression of Caspase-1 and GSDMD-N(P<0.01). ConclusionGupi Xiaoji Decoction can induce the pyroptosis of HepG2.2.15 cells by regulating Caspase-1 through multiple targets and multiple pathways.

ABSTRACT

ObjectiveTo predict the therapeutic target genes and related signaling pathways of Qinghuangsan (QHP) in the treatment of acute myeloid leukemia (AML) by network pharmacology,molecular docking,and further clarify its mechanisms through in vitro cell experiment. MethodThe active components and targets of QHP were retrieved from traditional Chinese medicine systems pharmacology database and analysis platform (TCMSP),traditional Chinese medicine integrated database (TCMID),TargetNet and SwissTargetPrediction databases,and AML-related target genes were obtained by GeneCards and online mendelian inheritance in man (OMIM) databases. After screening the common targets of QHP and AML,the protein-protein interaction (PPI) network of the common targets was constructed with STRING,followed by gene ontology (GO) term and Kyoto encyclopedia of genes and genomes (KEGG) pathway enrichment analysis based on RStudio software and clusterProfiler,Bioconductor packages. At the same time,Cytoscape software is used to construct the network of "disease-component-target" and "compound-target-pathway". Select the active ingredients of QHP for molecular docking with the top 8 targets in the "compound-target-pathway" network. In vitro cell experiment and Western blot were used to further verify the anti-AML effect of QHP. ResultThe prediction results show that there are 11 main active components of QHP,and 22 common targets of QHP and AML are collected. KEGG pathway analysis results show that phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) and mitogen-activated protein kinase (MAPK) signaling pathways may play a key role in the treatment of AML disease by QHP. "Compound-target-pathway" network analysis showed that the top 8 targets include Akt1,phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit alpha (PIK3CA),mitogen-activated protein kinase kinase 1 (MAP2K1),TP53,serine/threonine kinase (RAF1),B cell lymphoma(Bcl)-2,cysteine aspartic acid specific protease(Caspase)-9 and JUN. Molecular docking results showed that 3-indolyl-β-D-glucopyranoside was optimally docked with MAP2K1,isovitexin docked with PIK3CA,and indirubin docked with Bcl-2. Cell experiments show that 3-indolyl-β-D-glucopyranoside,isovitexin and indirubin can effectively inhibit the proliferation of AML cells,regulate the MAPK/PI3K signaling pathway,and inhibit the expression of Bcl-2 protein. ConclusionQHP can treat AML through "multi-component,multi-target,multi-pathway" synergistic treatment,and its mechanism of pharmacology may be related to the regulation of MAPK signaling pathway and PI3K/Akt signaling pathway.

ABSTRACT

To understand the current status of BMI of the elderly and related factors in longevity areas in China, and provide scientific evidence for the control of BMI level in elderly population. Data used in this study were obtained from Healthy Aging and Biomarkers Cohort Study, a sub-cohort of the Chinese Longitudinal Healthy Longevity Survey. A total of 2 825 elderly in 8 longevity areas in China were surveyed and measured in 2017. The BMI levels of 2 217 elderly aged 65 years and older were calculated and in follow up. The ordered classification logistic regression model was used to analyze the influencd factors for the BMI in the elderly. The BMI of the elderly in 8 longevity areas in China was (22.36±3.87) kg/m(2), and it was (22.76±3.58) kg/m(2) for males and (21.75±3.98) kg/m(2) for females. The BMI levels were normal in 1 165 elderly persons. The prevalence of underweight, overweight and obesity were 15.8, 24.0 and 7.7, respectively. Multivariate analysis showed that the main factors affecting the BMI of people under 100- years old were age (65-: =2.78, 95: 1.87-4.15; 80-: =1.47, 95: 1.00-2.17), smoking status (=0.46, 95: 0.32-0.66), annual household income (<30 000 Yuan: =1.26, 95: 1.07-1.47; 30 000-70 000 Yuan: =1.52, 95: 1.12-1.86), and frequency of tea intake(=1.36, 95: 1.01-1.71), while the factor in people aged ≥100 years was gender (=3.68, 95: 1.32-10.36). The prevalence of underweight, overweight and obesity were high in the elderly from longevity areas in China. It is necessary to pay attention to the trend of overweight and obesity due to smoking, higher annual household income and regular tea drinking in the elderly men.

Methods The epidemical data on NCP in Shanghai from January 20 to February 3, 2020 were collected for epidemiological descriptive analysis. Results The number of cumulative confirmed and suspected cases increased first and then decreased from January 20 to February 3, with the peak date being January 30 and January 29 respectively. The day-on-day growth rate of the suspected cases and the cumulative confirmed cases declined after January 27. Among the confirmed cases, the proportion of the exposure history of relevant confirmed cases was on the rise. The total number of confirmed cases of the resident population exceeded that of the population from other places to Shanghai, and Pudong new area had the largest number of confirmed cases. Conclusion The incidence of NCP showed a slowdown trend in shanghai, but it also faces the pressure of the peak of population returning to city, which should be paid enough attention to.

ABSTRACT

Objective To investigate the spatial-temporal distribution characteristics of varicella outbreak in Jiading District in Shanghai from 2015 to 2018. Methods Varicella epidemic report data was collected from the national system of disease control and prevention and analyzed by spatial-temporal scanning statistic methods. Results There were 5 889 varicella cases reported from the year 2015 to 2018, and the annual average incidence rate was 91.68 per 100 000.The incidence rate for children below 3 years old was found to be the highest, reaching 621.45 per 100 000, which was significantly higher than that for the group of 18 years old and above (χ2=16 616.788, P < 0.001).There were 5.41% and 5.31% of the cases in September and February, respectively, which were lower than other months, and the peak incidence occurred in December and November, accounting for 13.41% and 11.95%, respectively.The highest and lowest incidence rates were 151.80 per 100 000 occurring in central urban area and 59.89 per 100 000 in rural area, respectively.The spatial-temporal scanning showed that the low population density area had a wide cluster range and a low effect value.The high population density area has small cluster range and high effect value. Conclusion The incidence of vanicella presents a trend of population, seasonal and regional clustering.Therefore, targeted measure should be taken to prevent varicella in focus population.

ABSTRACT

Objective To evaluate the effects of water body environments on the microbial community of Oncomelania hupensis snails in marshlands of the eastern Dongting Lake where natural extinction of O. hupensis snails are found, so as to explore the correlation between the natural extinction of O. hupensis snails and the microbial community in snails. Methods Snails were caged water bodies in the Qianliang Lake marshland (Qianliang Lake regions) where natural extinction of snails was found and in the Junshan Park marshland (Junshan Park regions) in the eastern Dongting Lake for 30 days, and then all snails were collected and identified for survival or death. DNA sequencing of the fungi and bacteria was performed in snails before and after immersion in waters, and the biodiversity and abundance were analyzed. Results The survival rates of O. hupensis snails were 28.0% (70/250) and 64.8% (162/250) in Qianliang Lake regions and Junshan Park regions 30 days after immersion in waters, respectively (χ2 = 81.365, P < 0.01). The number of the fungal community and the biodiversity of the bacterial community were both greater in snails caged in Qianliang Lake regions post-immersion than pre-immersion, and there was a significant difference in the structure of the fungal and bacterial communities. The microbial community with a significant difference included Flavobacteriaceae,which was harmful to O. hupensis snails. Conclusion The water body environment affects the composition of the microbial community in O. hupensis snails in marshlands with natural snail distinction around the eastern Dongting Lake; however, further studies are required to investigate whether the natural distinction of snails is caused by water body environments-induced changes of the microbial spectrum in O. hupensis snails.

ABSTRACT

Objective@#To summarize the nursing experience of a case of pseudomonas aeruginosa septic shock with massive gangrene in infants, and to explore the nursing method of pseudomonas aeruginosa septic shock with massive gangrene in infants.@*Methods@#Shock was treated by close observation, establishment of effective venous channels and effective dilatation; daily infusion of virus inactivated plasma supplemental immunoglobulin; planned detoxification and dressing change, as well as effective management of stool, urine and body position enable gangrene to be effectively controlled;At the same time, necessary nutrition and psychological support were given to the children.@*Results@#After 15 days of careful nursing, the wound surface recovered well and the vital signs were stable. Pediculated skin grafting may be performed at a scheduled time.@*Conclusions@#The practice of this case proved that the above nursing measures were effective for pseudomonas aeruginosa septic shock with massive gangrene.

ABSTRACT

Schistosomiasis is a modern disease name, but it has been widespread in ancient China and exists in a specific form in traditional Chinese medicine (TCM) . The purpose of the paper is to make clear the existing form of schistosomiasis in TCM and infer the prevalence of schistosomiasis in ancient China. The paper focuses on the period when great progress was made on schistosomiasis by TCM, and sums up the understanding of TCM toward schistosomiasis in this period. By studying and analyzing the literature of schistosomiasis in this period, the paper tries to find out the representative Chinese medicine symptom description and TCM "other name" of schistosomiasis, so as to accurately judge whether the relevant description in ancient TCM books and historical documents can provide scientific basis for schistosomiasis. It is important to understand the prevalence and cognition of schistosomiasis in ancient China.

ABSTRACT

Objective To investigate the reasons of natural extinction of Oncomelania hupensis snails by comparing the difference of the water chemical properties between the snail-breeding marshland and non-snail marshland in Eastern Dongting Lake areas. Methods Three adjoining marshlands in Eastern Dongting Lake areas were selected, one was a snail-breeding marshland, the second was a non-snail marshland, and the third was a boundary zone between them. During the periods of water-rising season, wet season and water-falling season, the water samples from the bottom of 0.5 m were collected by the systematic random sampling method (200 m × 200 m grid method). In addition, the water samples below the surface of 0.5 m were also collected in the wet season. The physicochemical indexes of the sampling water were detected. Results According to the Surface Water Environmental Quality Standard (GB 3838–2002), the water quality indicators which were less than the detection limit were Cr, Cd, Zn, Hg, Cu, S- and CN- during the three sampling seasons. The pollutants which were slightly higher than Grade III but lower than Grade IV (Hg ≤ 1 μg/L, TP ≤ 0.1 mg/L) were Hg and TP. The pollutants below the Grade III included As and F-. The temperature and pH values were within the Grade III. The oxygen consumption indicators covering DO, COD and BOD in the water-rising season and wet season were within the Grade III, while the value of BOD was beyond the scope of Grade III but was between Grade Ⅳ (≤ 6 mg/L) and Grade V (≤ 10 mg/L) in the water-falling season. The differences of the water pollution indexes between the snail-breeding marshland and non-snail marshland were statistically significant, and these indexes included F-, As and pH in the water-rising season (P < 0.05); pH, BOD and Fat the surface layer in the wet season (P < 0.05); F- at the bottom in the wet season (P < 0.05); TP and F- in the water-falling season. Moreover, the above-mentioned indexes in the non-snail marshland of Qianliang Lake were higher than those in the snail-breeding marshland of Junshan Park. Thus, F- was the only index which had statistical differences in every sampling season, and the concentration of F- at the non-snail marshland was also higher than that at the snail-breeding marshland. In the water-rising season, the pH value at the non-snail marshland exceeded the suitable range for the growth of snails (6.8 to 7.8). Conclusion In the Eastern Dongting Lake area, the high concentration of F- and pH of water may be responsible for the natural extinction of O. hupensis snails.

ABSTRACT

Schistosomiasis is one of the key diseases of surveillance and prevention in China. The elimination of schistosomiasis is of great significance to people’s health and social economy. With the development of spatial epidemiology, progress has been made in the spatial distribution of schistosomiasis, the prediction of spatial and temporal trends, and analysis of the environmental factors. This paper reviews the application of spatial epidemiology in the control and prevention of schistosomiasis and introduces the spatio-temporal distribution methods, spatial model, and application of remote sensing technology.

Objective:To investigate the expression of activator of basal transcription 1 (ABT1) protein in gastric cancer tissue and its relationships with the clinical parameters and prognosis of gastric cancer patients, and to clarify the role of ABT1in the occurrence and development of gastric cancer.Methods:A total of 100cases of cancer tissue of the gastric cancer patients and 80pairs of adjacent tissue were selected.The expressions of ABT1in cancer tissue and adjacent tissue were detected by immunohistochemistry and the proportion of stained cells and the degree of staining in the immunohistochemistry results were analyzed using semi-quantitative analysis.The relationships between the semi-quantitative analysis results and the clinical parameters of gastric cancer patients were statistically analyzed.Kaplan-Meier method was used to analyze the correlation between the ABT1 protein expression level and the survival of gastric cancer patients.Results:ABT1-positive staining was observed in the nucleus and cytoplasm of gastric cancer tissue and adjacent gastric tissue.The expression level of ABT1in gastric cancer tissue was lower than that in adjacent tissue (P=0.021) .The ABT1protein expression level in gastric cancer tissue was significantly negatively correlated with the pathological grade (r=-0.224, P=0.026) .The Kaplan-Meier analysis results of the survival curve showed that the high expression of ABT1was associated with good prognosis in the gastric cancer patients (HR=1.483, P<0.01) .The survival rate of gastric cancer patients with high ABT1expression was significantly higher than that of the patients with low ABT1expression (HR=2.411, P=0.0272) .Conclusion:The expression of ABT1in gastric cancer tissue is lower, indicating that ABT1can be used one of the markers of good prognosis of gastric cancer.

Abstract  B cell maturation antigen (BCMA) is an ideal target for precise treatment due to its highly selective expression on malignant myeloma cells. This review summarizes briefly the advances in the latest research progress on biological activity of BCMA, its significance as a biomarker and immunotherapy direcited against BCMA, such as bispecific antibodies, antibody drug conjugates, chimeric antigen receptor T cell therapy against mature B cell antigens.

ABSTRACT

Schistosomiasis is a modern disease name, but it has been widespread in ancient China and exists in a specific form in traditional Chinese medicine (TCM) . The purpose of the paper is to make clear the existing form of schistosomiasis in TCM and infer the prevalence of schistosomiasis in ancient China. The paper focuses on the period when great progress was made on schistosomiasis by TCM, and sums up the understanding of TCM toward schistosomiasis in this period. By studying and analyzing the literature of schistosomiasis in this period, the paper tries to find out the representative Chinese medicine symptom description and TCM "other name" of schistosomiasis, so as to accurately judge whether the relevant description in ancient TCM books and historical documents can provide scientific basis for schistosomiasis. It is important to understand the prevalence and cognition of schistosomiasis in ancient China.

ABSTRACT

Objective To construct sensitive,scientific,targeted index evaluation system of psychiatric nursing quality and provide basis for evaluation and surveillance of psychiatric nursing quality.Methods Based on 3-di-mension quality construction theory,literature analysis and experts group discussion were adopted to preliminarily screen indexes and compile inquiry questionnaire.Two rounds of expert consultation were performed using Delphi technique,and sensitive index evaluation system of psychiatric nursing quality was revised and finalized.Results Positive coefficients of two rounds of expert consultation were 90.63% and 96.55%,authority coefficients were 0.81 and 0.83,and coordination coefficients were 0.136-0.335;the constructed sensitive index evaluation system of psychiatric nursing quality included 3 first-class indicators and 18 second-class indicators.Conclusion The constructed sensitive index evaluation system of psychiatric nursing quality has psychiatric characteristics.Its effects on nursing quality control and evaluation can be verified from clinical practices,then effective data sharing can be gradually realized.
